Paracetamol  (acetaminophen in the United States) is an  analgesic  and  antipyretic  without antiinflammatory activity. Its mechanisms of action remain incompletely understood, but it has a highly selective inhibitory effect on COX - 2 in the spinal cord by reducing the availability of an essential co - substrate for the enzymatic action of COX - 2. This inhibits the transmission of pain impulses to higher centres.
